Пандемия COVID-19 увеличивает случаи отдаленных нежелательных явлений заболевания. Отягощенный по сердечно-сосудистым заболеваниям анамнез диктует необходимость изучения системы гемостаза, в особенности у пациентов с гипертонической болезнью. Цель – изучение микроциркуляции у пациентов с артериальной гипертензией, перенесших COVID-19. Материалы и методы. Отобраны пациенты, получающие лечение в медицинских учреждениях г. Самары с января по март 2021 г. Проанализированы две группы пациентов с подтвержденной COVID-19 (исследование биологического материала методом полимеразно-цепной реакции): 167 пациентов с отягощенным по гипертонической болезни анамнезом и 68 пациентов контрольной группы, отрицающие эпизоды повышения артериального давления. Основные характеристики микроциркуляции оценивали с помощью метода лазерной допплеровской флоуметрии (ЛДФ) сосудов кожи. Результаты. Средний возраст пациентов, перенесших COVID-19, составил 52,3±14,2 года, n=86 (51,4 %) – мужского пола. Большинство пациентов (n=61/36,5 %) принадлежали к возрастной группе от 45 до 54 лет. Пятая часть пациентов (n=35/20,9 %) не проявляли жалоб по основному заболеванию, у остальных среди клинических проявлений зафиксированы общая слабость, повышение температуры тела, одышка, кашель, головные боли, нестабильное артериальное давление, воспаление ЛОР-органов. Сравнивая показатели микроциркуляции у исследуемых пациентов с контрольной группой, зафиксировано снижение показателей перфузии, что требует принятия мер с целью профилактики ранних и отдаленных микро- и макротромбозов. Заключение. Изучение особенностей микроциркуляции методом ЛДФ сосудов кожи выявило патологический тип с преобладанием вазоконстрикций у пациентов с отягощенным по гипертонической болезни анамнезом. Учитывая доказанное увеличение частоты случаев тромботических явлений у пациентов, перенесших COVID-19, зафиксированные изменения микроциркуляции требуют нового подхода к профилактике нежелательных явлений в отношении процесса гиперкоагуляции, в особенности у пациентов с отягощенным сердечно-сосудистым анамнезом.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. The COVID-19 pandemic multiplies the incidence of long-term adverse events. Family history of heart disease predicates the necessity of study hemostasis in hypertensive patients. The purpose was to study the microcirculation in hypertensive COVID-19 survivors. Materials and methods. We selected patients treated at the hospitals of Samara from January to March 2021. COVID-19 survivors (diagnosis was confirmed by polymerase chain reaction analysis) were divided into 2 groups and analysed: the group with family history of hypertension (167 patients) and control group with no relevant past medical history (68 patients). The main microcirculation characteristics were evaluated by laser Doppler flowmetry of skin blood flow. Results. The average age of COVID-19 survivors was 52.3±14.2 years, n=86 (51.4 %) – male. The majority of patients (n=61/36.5 %) were 45 to 54 years. A fifth of the patients (n=35/20.9 %) showed no complaints about the underlying disease, the clinical manifestations of the rest were general weakness, fever, shortness of breath, cough, headaches, unstable blood pressure, inflammation of the ENT organs. By comparison the microcirculation parameters of the case patients and control group, a decreased perfusion was detected. It requires a preventive medicine to prevent from early and distant micro and macrothrombosis. Conclusion. The study of microcirculation essential features by the LDF of skin blood flow revealed a pathological type with a predominance of vasoconstrictions in patients with a history of hypertension.
