В исследованиях, выполненных на значительных выборках взрослых пациентов с врожденными пороками сердца (ВПС), была отмечена высокая распространенность тромбоцитопении, особенно среди больных с синдромом Эйзенменгера (сЭйз). Точные механизмы снижения числа тромбоцитов у взрослых с ВПС в настоящее время неизвестны. Цель - оценить клиническое значение тромбоцитопении у беременных с ВПС и сделать заключение о возможных причинах ее развития. Материал и методы. Проведено ретроспективное описательное исследование 17 беременных с различными ВПС, поступивших в Специализированный перинатальный центр при СЗФМИЦ им. В. А. Алмазова для родоразрешения. Данные представлены в виде медиана (25-й; 75-й процентиль), при нормальном распределении - как М±σ. Результаты исследования. Медиана возраста составила 26 (21; 28) лет. Средний срок гестации на момент поступления в стационар - 31 (28; 34) нед. У 12 из 17 пациенток на момент поступления сформировался сЭйз. При поступлении расчетное ДЛАсис составляло 88 (82; 98) мм рт. ст., а SpO2 - 92 (85; 95) %. В общей выборке медиана числа тромбоцитов составила 105 (62; 164)·103/мкл, у 12 (71 %) женщин этот показатель был менее 150·103/мкл. Выполненное клинико-лабораторное исследование не выявило у пациенток критериев гемобластозов, лекарственно-индуцированной или иммунной тромбоцитопении, не было признаков антифосфолипидного синдрома и «НеПр»-синдрома. Обнаружено достоверное различие между числом тромбоцитов у пациенток с сЭйз и женщин без шунтирования крови справа налево: 99 (73 ; 105) и 162 (138 ; 166)·103/мкл, соответственно, p&lt;0,001. Средний объем тромбоцитов у пациенток с сЭйз был выше, чем у женщин с ВПС без шунта справа налево: 12,2±2,4 и 10,6±1,6 фл соответственно, p=0,003. Кроме того, была обнаружена достоверная корреляционная связь с коэффициентом - 0,52 между числом тромбоцитов и их средним объемом. После выполненных операций кесарева сечения скончались три пациентки с сЭйз. Таким образом, летальность во всей выборке составила 17,6 %, а среди женщин с сЭйз - 25 %. Выводы. Снижение числа тромбоцитов у взрослых пациентов с синдромом Эйзенменгера имеет комплексный характер и связано с повышенным потреблением тромбоцитов, тяжелой гипоксемией и шунтированием крови справа налево со снижением тромбоцитопоэза в капиллярном русле малого круга.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. The large-scale studies on adult patients with congenital heart disease (CHD) demonstrated high prevalence of thrombocytopenia, especially in patients with Eisenmenger syndrome (sEis). The exact reasons for the decline of platelet count in adults with CHD are currently unknown. Objective: to determine the prevalence of thrombocytopenia in a representative group of adult patients with CHD and explore the possible causes of thrombocytopenia. Methods. A retrospective study of 17 pregnant women with CHD was carried out. All women were admitted for delivery to Federal Almazov North-West Medical Research Centre. Data are presented as median (25th, 75th percentile) and as M±σ for parametric variables. Results. The median age was 26 (21; 28) years. The average period of gestation at the time of admission was 31 (28; 34) weeks. 12 (70.6 %) patients had sEis. At the time of admission PAPsys was 88 (82; 98) mm Hg, SpO2 - 92 (85; 95)%. In whole group the platelet count median was 105 (62; 164) 109/L, while in 12 (71%) women platelet count was less than 150 109/L. There was a significant difference between patients with sEis and women without right-to-left shunt in platelet count: 99 (73; 105) and 162 (138; 166) 109/L, respectively, p&lt;0.001. Mean platelet volume (MPV) in patients with sEis was higher than in women without right-to-left shunt: 12.2±2.4 fl and 10.6±1.6 fl, respectively, p=0.003. Furthermore, a significant negative correlation (r=-0.52) was found between platelet count and MPV. Three patients with sEis died after cesarean section. Thus, the mortality in the entire group was 17.6%, and among women with sEis - 25%. Conclusions. Reduced platelet count is typical for adult patients with sEis.
