Поиск генетических детерминант развития сердечно-сосудистых заболеваний занимает важное место в концепции персонализированной медицины. Цель. Оценить влияние полиморфизма Arg72Pro (rs1042522) гена ТP53 на отдаленный прогноз у пациентов c ишемической болезнью сердца (ИБС), подвергшихся аортокоронарному шунтированию (АКШ). Материалы и методы. В исследование было включено 89 пациентов со стабильной ИБС и многососудистым поражением коронарных артерий (КА). Первая выборка – 61 человек, с вариантами Аrg/Arg и Arg/Pro. Вторая – 28 человек с вариантом Pro/Pro. У пациентов оценивалось ремоделирование миокарда, качество жизни больных, а также клинические данные за 18 месяцев наблюдения. Результаты. У больных с вариантами Аrg/Arg и Arg/Pro гена ТР53 за период наблюдения произошло увеличение конечно-диастолического объема (КДО) и конечно-систолического объема (КСО) левого желудочка (ЛЖ) в I выборке: 104,4±4,2 мл и 45,8±3,5 мл соответственно, через год после АКШ – 113,3±5,6 мл и 52,4±4,7 мл (p&lt;0,05). Подобная отрицательная динамика КДО и КСО ЛЖ во II группе не отмечалась. Качество жизни по Миннесотской шкале до АКШ в I когорте было 27,3±2,4 балла, а через 18 месяцев – 22,6±2,1 (р=0,126). Во II группе – 32,8±1,9, а на отдаленном этапе наблюдения – 20,9±2,2 балла (р&lt;0,001). Острая декомпенсация хронической сердечной недостаточности (ОДХСН) в отдаленном периоде наблюдения в I группе развилась у 19 (32,2 %) больных, тогда как во II – у 3 (10,7 %) (ОШ 3,96; 95 % ДИ: 1,06–14,76; р=0,036). Заключение. У пациентов с вариантами Arg/Arg и Arg/Pro гена TP53 отмечается прогрессирование ремоделирования ЛЖ, а также в 4 раза повышенный риск развития ОДХСН в течение 18 месяцев после АКШ. Среди больных с аллелем Pro качество жизни после АКШ улучшается.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Investigation for genetic determinants of cardiovascular diseases takes an important place in the concept of personalized medicine. Objective. To evaluate the effect of Arg72Pro (rs1042522) polymorphism TP53 gene on long-term prognosis in patients with coronary artery disease (CAD) undergoing coronary artery bypass grafting (CABG). Materials and methods. 89 patients with stable CAD and multivessel coronary artery disease (CA) were included in the study. The first sample was 61 people who were carriers of the Arg/Arg and Arg/Pro variants. The second one was 28 people who had the Pro/Pro variant. Patients were evaluated for myocardial remodeling, quality of life, as well as clinical data for the 18-months follow-up period. Results. In carriers of the Arg/Arg and Arg/Pro variants of the TP53 gene, during the follow-up period, we obtained elevation of end-diastolic volume (EDV) and end-systolic volume (ESV) of left ventricle (LV) in the first sample: 104.4 ± 4.2 ml and 45.8±3.5 ml, one year after CABG – 113.3±5.6 ml and 52.4±4.7 ml respectively (p&lt;0.05). Negative dynamics of EDV and LV ESV in patients of group II were not observed. Quality of life by Minnesota scale before CABG in cohort I was 27.3±2.4 points, in 18 months after – 22.6±2.1 (p=0.126). In group II: 32.8±1.9, 1,5 years later – 20.9±2.2 points (p&lt;0.001). Acute decompensation of chronic heart failure (ADHF) in the long-term follow-up period in group I occurred in 19 (32.2 %) patients, while in group II – in 3 persons (10.7 %) (OR 3.96; 95 % CI: 1.06-14.76; p=0.036). Conclusion. Patients with the Arg/Arg and Arg/Pro variants of the TP53 gene demonstrated progression of LV remodeling, as well as a 4-fold increased risk of ADHF within 18 months after CABG.
