Лечение пациентов с ОАСНК направлено на облегчение симптомов хронической ишемии нижних конечностей и профилактику основных сердечно-сосудистых осложнений атеросклероза. В статье рассматривается алгоритм ведения пациентов с ОАСНК, подчеркивается необходимость эффективной терапии перемежающейся хромоты. Рассмотрены причины ее возникновения, обсуждаются исследования фармакологических средств, используемых для ее лечения. В обзоре проанализированы результаты опубликованных клинических исследований влияния пропионил-L-карнитина на выраженность перемежающейся хромоты, оценена безопасность его использования. С учетом того, что терапевтические дозы пропионил-L-карнитина имеют хороший профиль безопасности и оказывают положительное влияние при ряде сопутствующих ОАСНК заболеваний, его назначение пациентам с симптомом перемежающейся хромоты следует признать особенно целесообразным. Хотя прием пропионил-Lкарнитина, по-видимому, безопасен, долгосрочных доказательств его безвредности в настоящее время нет, что требует дальнейшего изучения этого препарата.</p></abstract><trans-abstract xml:lang="en"><p>Lower extremity peripheral arterial disease (PAD) is a manifestation of systemic atherosclerosis and is recognized by the ankle-brachial index (ABI). PAD patients’ treatment addresses the relief of chronic leg ischemia and prevention of cardiovascular complications of atherosclerosis. The review describes the PAD patients’ management, and highlights the necessity of intermittent claudication effective treatment with regard to its causes. We discuss the medications used for its treatment and analyze the results of reported clinical trials of propionyl-L-carnitine effect on intermittent claudication signs and evaluated its safety. Since the therapeutic doses of propionyl-L-carnitine are safe and have a positive effect on PAD patients with cardiovascular risks, it should be considered to have high clinical relevance in case of intermittent claudication.
