Спондилоартрит (СпА), ассоциированный с воспалительным заболеванием кишечника (ВЗК) – заболевание из группы СпА, развивающееся у пациентов с болезнью Крона (БК) и язвенным колитом. Нарушения кишечно-сосудистого барьера, включающие повышение проницаемости эпителия и повреждение эндотелиального гликокаликса (ЭГк), продемонстрированы как при БК, так и при аксиальном СпА (АксСпА), и могут служить патогенетической основой совместного развития и прогрессирования этих заболеваний.</p></sec><sec><title>Цель</title><p>Цель. Оценить значимость исследования маркеров повреждения ЭГк и проницаемости эпителия у пациентов с БК-ассоциированным АксСпА.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследованы 22 пациента с АксСпА, ассоциированным с БК (группа А), 29 пациентов с АксСпА без ВЗК (группа Б), 27 пациентов с БК (группа В), 28 условно здоровых лиц (группа Г). Исследовались кальпротектин (ФК) и зонулин (ФЗ) в кале, гиалуронан и синдекан 1 в сыворотке. Область пограничной перфузии (PBR) и Индекс здоровья (MVHS) микрососудов измеряли темнопольной микроскопией сублингвальной области.</p></sec><sec><title>Результаты</title><p>Результаты. У пациентов с БК-ассоциированным АксСпА выявлено увеличение PBR (p&lt;0,001) и снижение MVHS (p=0,001) в сравнении со здоровыми лицами. Только у пациентов с БК выявлено снижение гиалуронана сыворотки (p=0,006), ассоциированное с колитом и глубокими язвами при эндоскопии. Увеличение PBR позволяло выявить очень высокую активность АксСпА в группе А с чувствительностью 100 % и специфичностью 83,3 %. У пациентов группы А обнаружена корреляция гиалуронана с ФК (ρ=–0,541; p=0,030). Построено дерево классификации, включающее ФК, ФЗ, гиалуронан и MVHS, которое позволяет определить наличие БК у пациентов с АксСпА с точностью 90,2 %.</p></sec><sec><title>Выводы</title><p>Выводы. Исследование маркеров повреждения кишечно-сосудистого барьера позволяет улучшить методы диагностики и оценки интегральной активности АксСпА, ассоциированного с БК.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Spondyloarthritis (SpA) associated with inflammatory bowel disease (IBD) is a disease in the SpA group developing in patients with Crohn’s disease (CD) and ulcerative colitis. Gut-vascular barrier impairments, including increased epithelial permeability and endothelial glycocalyx (EGc) damage, have been demonstrated in both CD and axial SpA (axSpA), and can serve as a pathogenetic basis for the joint development and progression of these diseases.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the significance of EGc damage and epithelial permeability markers in patients with CD-associated axSpA.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We examined 22 patients with axSpA associated with CD (group A), 29 patients with axSpA without IBD (group B), 27 patients with CD (group C) and 28 conditionally healthy controls (group D). Calprotectin (FC) and zonulin (FZ) in feces, hyaluronan and syndecan 1 in serum were studied. Perfusion boundary region (PBR) and the Microvascular Health Index (MVHS) were measured by dark-field microscopy in the sublingual region.</p></sec><sec><title>Results</title><p>Results. In patients with CD-associated axSpA, an increase in PBR (p&lt;0.001) and a decrease in MVHS (p=0.001) were revealed in comparison with healthy individuals. Only CD patients revealed decreased serum hyaluronan (p=0.006) associated with colitis and deep ulcers on endoscopy. Increased PBR allowed to identify very high axSpA activity in group A with a sensitivity of 100 % and specificity of 83.3 %. In group A patients, a correlation between hyaluronan and FC was found (ρ=–0.541; p=0.030). A classification tree, including FC, FZ, hyaluronan, and MVHS, was constructed to determine the presence of CD in axSpA patients with an accuracy of 90.2 %.</p></sec><sec><title>Conclusions</title><p>Conclusions.
