Значительное количество публикаций, посвященных анатомической вариации магистральных артерий плеча, используют результаты патологоанатомических наблюдений или данные ретроспективного анализа. Описание редких вариантов развития магистральных артерий является актуальной задачей, позволяющей разрабатывать и совершенствовать методы хирургического лечения с учетом индивидуальных особенностей пациентов. Цель – представить подробное описание выявленного варианта развития артерий правой верхней конечности; провести сравнение особенностей гемодинамики со второй конечностью пациента и результатами, полученными при обследовании контрольной группы. Материалы и методы. Приводится описание клинического случая нетипичного варианта развития артерий верхней конечности. Данные допплерографии сравнивались с результатами обследования пятидесяти относительно здоровых пациентов обоих полов в возрасте от 32 до 85 лет. Проведен статистический анализ с использованием IBM SPSS Statistics 20. Результаты. У женщины 46 лет диагностировано разделение подмышечной артерии в области верхней трети плеча на два артериальных ствола. Их диаметр был 2,3 мм, они проходили в медиальной межмышечной борозде плеча и в дальнейшем самостоятельно продолжались в лучевую и локтевую артерии. При оценке кровоснабжения ладонных дуг определяются признаки магистрального артериального кровотока, при проведении пробы Аллена отмечена доминирующая роль локтевой артерии в кровоснабжении кисти. Обсуждение. Полученные результаты у пациентки с необычным вариантом развития артерий верхней конечности позволяют говорить о тенденции к повышению индексов резистивности и пиковой скорости кровотока в сравнении с данными контрольной группы. В целом проведенное исследование признаков значимых гемодинамических изменений не выявило. Выводы. Полученные результаты позволяют говорить о тенденции к повышению индексов резистивности и пиковой скорости кровотока. Представленный случай указывает, что своевременное выявление гемодинамических нарушений позволяет активно использовать методы обходного шунтирования для восстановления или улучшения развившихся изменений кровоснабжения, выбирать оптимальный уровень для проведения реконструктивного вмешательства.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. A significant number of publications devoted to anatomical variation of the main arteries of the shoulder use the results of pathoanatomical observations or retrospective analysis data. The description of rare variants of the development of the main arteries is an urgent task that allows to develop and improve surgical treatment methods taking into account the individual characteristics of patients. Aim. to provide a detailed description of the identified variant of the development of the arteries of the right upper limb. To compare the hemodynamic features with the patient’s second limb and the results obtained during the examination of the control group. Materials and methods. A clinical case of an unusual variant of the development of the arteries of the upper limb is described. The Dopplerography data were compared with the results of an examination of 50 relatively healthy patients of both sexes aged 32 to 85 years. Statistical analysis was performed using IBM SPSS Statistics 20. Results. A 46-year-old woman was diagnosed with a split of the axillary artery in the upper third of the shoulder into two arterial trunks. Their diameter was 2.3 mm, they passed through the medial intermuscular groove of the shoulder and then continued independently into the radial and ulnar arteries. When assessing the blood supply to the palmar arches, signs of arterial blood flow are determined. During the Allen test, the dominant role of the ulnar artery in the blood supply to the hand was noted. Discussion. The results obtained in the female patient with an unusual variant of the development of the arteries of the upper extremity suggest a tendency to increase the resistance indices and peak blood flow velocity in comparison with the data of the control group. In general, the study revealed no signs of significant hemodynamic changes. Conclusions. The results obtained suggest a tendency towards an increase in resistance indices and peak blood flow velocity.
