Выделяют два основных механизма возникновения феномена невосстановления кровотока, возникающих у пациентов с инфарктом миокарда после первичной чрескожной ангиопластики: проявления ишемического-реперфузионного повреждения микроциркуляторного русла миокарда и эмболизация дистального артериального русла фрагментами тромба и атеросклеротической бляшки. В обзоре описаны факторы, которые способствуют появлению феномена постишемического невосстановления кровотока, механизмы его появления, принципы диагностики и терапевтические мишени.</p></abstract><trans-abstract xml:lang="en"><p>The phenomenon of «no-reflow» refers to a state of myocardial tissue hypoperfusion or absence of perfusion after eliminating the cause of the occlusion of the coronary artery. Two main pathogenetic mechanisms may contribute to the development of no-reflow phenomenon after primary percutaneous coronary intervention in patients with myocardial infarction: ischemic-reperfusion injury of the microcirculatory vessels and embolization of the distal arterial bed with fragments of thrombus and atherosclerotic plaque.
