Выраженность ишемии нижних конечностей может быть определена на основании уровня лодыжечноплечевого индекса (ЛПИ), что является неинвазивным и доступным способом диагностики. Одним из способов, позволяющих оценить кровоснабжение нижних конечностей не только в покое, но и при физической нагрузке, является транскутанная оксиметрия (ТКО), что обуславливает интерес к изучению возможности определения выраженности хронической ишемии нижних конечностей с использованием этого метода.</p></sec><sec><title>Цель</title><p>Цель. Определить корреляцию между показателями чрескожного напряжения кислорода (tcpO2) на стопах при пробе с физической нагрузкой и уровнем ЛПИ в покое у больных с окклюзионно-стенотическим поражением артерий нижних конечностей.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В ретроспективное исследование включены 34 пациента (23 мужчины и 11 женщин, средний возраст 69±4,9 лет) с окклюзионно-стенотическим поражением аорто-подвздошного сегмента и артерий нижних конечностей. У всех больных была диагностирована хроническая ишемия IIб стадии по Фонтейну – Покровскому. Пациентам были выполнены такие исследования как: мультиспиральная компьютерно-томографическая ангиография и/или катетерная ангиография брюшной аорты и артерий нижних конечностей, ТКО стоп при пробе с физической нагрузкой, вычисление ЛПИ. Критериями исключения являлись: наличие окклюзионно-стенотического поражения артерий голени, препятствующего измерению лодыжечного давления; острая или хроническая ишемия, угрожающая потерей нижней конечности; наличие сопутствующих заболеваний, ограничивающих проведение тредмил-теста. Оценивали корреляцию между уровнем ЛПИ и минимальными значениями измеренного tcpO2 на стопах в состоянии покоя сидя и стоя, во время и после ходьбы на беговой дорожке, а также отношениями их значений в разные периоды измерения. Проведен анализ предсказательной способности показателей ТКО в отношении уровня ЛПИ.</p></sec><sec><title>Результаты</title><p>Результаты. Не установлено статистически значимой корреляции между абсолютными значениями tcpO2 на стопах и ЛПИ. Максимальный статистически значимый (p=0,001) коэффициент корреляции Спирмена (rho=0,48) оказался между ЛПИ и отношением tcpO2 на стопах в восстановительном периоде после физической нагрузки к исходному значению этого показателя, измеренного в положении стоя до нагрузки. При уровне указанного индекса менее 0,84 чувствительность и специфичность в отношении снижения ЛПИ менее 0,7 составляет 70 % и 88 %, соответственно (p&lt;0,001).</p></sec><sec><title>Заключение</title><p>Заключение. Отношение показателей tcpO2 на стопах в восстановительном периоде после физической нагрузки к аналогичному показателю, измеренному в положении стоя до нагрузки, достоверно коррелирует со значением ЛПИ, а уровень указанного отношения менее 0,84 может свидетельствовать о субкомпенсированной ишемии нижних конечностей.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The severity of lower extremity ischemia can be determined based on the ankle-brachial index (ABI), a noninvasive and widely available diagnostic method. Transcutaneous oximetry (tcpO2) is one of the methods for assessing lower extremity blood flow not only at rest but also during exercise. This has prompted interest in studying the feasibility of assessing the severity of chronic lower extremity ischemia using this method.</p></sec><sec><title>Objective</title><p>Objective. To determine the correlation between transcutaneous oxygen tension (tcpO2) values on the feet during exercise testing and the ABI value at rest in patients with occlusive-stenotic lesions of the lower extremity arteries.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. This retrospective study included 34 patients (23 men and 11 women, mean age 69±4.9 years) with occlusive-stenotic lesions of the aortoiliac segment and lower extremity arteries. All patients were diagnosed with chronic ischemia stage IIb according to Fontaine-Pokrovsky. The patients underwent the following examinations: multislice computed tomography angiography and/or catheter angiography of the abdominal aorta and lower extremity arteries, tcpO2 of the feet during exercise testing, and ABI calculation. Exclusion criteria were occlusivestenotic lesions of the lower extremity arteries preventing ankle pressure measurement, acute or chronic limb-threatening ischemia, and comorbidities limiting treadmill testing. We assessed the correlation between the ABI level and minimum foot tcpO2 values at rest, measured sitting, standing, during and after treadmill walking, as well as their ratios at different measurement periods. The predictive ability of tcpO2 indicators in relation to the ABI level was also analyzed.</p></sec><sec><title>Results</title><p>Results. No statistically significant correlation was found between absolute foot tcpO2 values and ABI. The highest statistically significant (p=0.001) Spearman correlation coefficient (rho=0.48) was found between ABI and the ratio of foot tcpO2 during the recovery period after exercise to the baseline value measured in a standing position before exercise. At a ratio below 0.84, the sensitivity and specificity for a decrease in ABI below 0.7 were 70% and 88%, respectively (p&lt;0.001).</p></sec><sec><title>Conclusion</title><p>Conclusion.
