Циклофосфамид-индуцированная кардиотоксичность является одним из наиболее тяжелых осложнений высокодозной иммуносупрессивной терапии с аутологичной трансплантацией гемопоэтических стволовых клеток (ВИСТ-АТГСК). Ранние сроки развития миокардиальной дисфункции (2–10 сутки) ограничивают эффективность существующих подходов к мониторингу и требуют разработки методов Раннего прогнозирования.</p><p>Цель – разработка прогностических моделей для Ранней идентификации пациентов с высоким риском дисфункции миокарда при ВИСТАТГСК.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В обсервационное когортное исследование включены 74 пациента с рассеянным склерозом (средний возраст 36±8 лет), которым проводилась ВИСТ-АТГСК с применением высоких доз циклофосфамида. Обследование выполняли до лечения, на третьи и седьмые сутки терапии. Оценивали кардиоспецифические биомаркеры, электрокардиографические и эхокардиографические параметры. Для построения прогностических моделей использовали пошаговый дискриминантный анализ.</p></sec><sec><title>Результаты</title><p>Результаты. Субклиническая кардиотоксичность (повышение NT-proBNP ≥534 пг/мл) выявлена у 65 % пациентов, клинически манифестная – у 5 %. Построена модель прогнозирования субклинической дисфункции миокарда, включающая показатели диастолической функции (E/A, ΔE/A, ΔE), объем левого предсердия и уровень гемоглобина (чувствительность 96,8 %, точность 88,1 %). Модель прогнозирования острой левожелудочковой сердечной недостаточности основана на значениях E/e’, уровне мочевины и динамике массы миокарда левого желудочка (общая точность 97,3 %). Ключевые изменения выявляются уже на третий день терапии.</p></sec><sec><title>Заключение</title><p>Заключение. Разработанные модели позволяют на Раннем этапе ВИСТ-АТГСК идентифицировать пациентов с высоким риском кардиотоксичности. Это создает возможность своевременной коррекции терапии и профилактики тяжелых сердечно-сосудистых осложнений.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Cyclophosphamide-induced cardiotoxicity is one of the most severe complications of high-dose immunosuppressive therapy followed by autologous hematopoietic stem cell transplantation (HDIT-AHSCT). The early onset of myocardial dysfunction (within 2–10 days) limits the effectiveness of current monitoring strategies and necessitates the development of early prediction methods.</p></sec><sec><title>Objective</title><p>Objective. To develop predictive models for early identification of patients at high risk of myocardial dysfunction during autologous HDIT-AHSCT.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. This observational cohort study included 74 patients with multiple sclerosis (mean age 36±8 years) undergoing autologous HDIT-AHSCT with high-dose cyclophosphamide. Assessments were performed before treatment and on days 3 and 7 of therapy. Cardiac biomarkers, electrocardiographic, and echocardiographic parameters were evaluated. Stepwise discriminant analysis was used to develop predictive models.</p></sec><sec><title>Results</title><p>Results. Subclinical cardiotoxicity (NT-proBNP ≥534 pg/mL) was detected in 65 % of patients, while clinically manifest cardiotoxicity occurred in 5 %. A predictive model for subclinical myocardial dysfunction was developed, incorporating diastolic function parameters (E/A, ΔE/A, ΔE), left atrial volume, and hemoglobin level (sensitivity 96.8 %, accuracy 88.1 %). A model for predicting acute left ventricular heart failure was based on E/e’, serum urea levels, and changes in left ventricular mass (overall accuracy 97.3 %). Key alterations were detectable as early as day 3 of therapy.</p></sec><sec><title>Conclusion</title><p>Conclusion. The proposed models enable early identification of patients at high risk of cardiotoxicity during autologous HDIT-AHSCT.
