В исследование были включены 70 пациентов с атеросклерозом артерий каротидного бассейна, 43 мужчины и 27 женщин. Средний возраст пациентов составлял (61,1±8,54) года. Всем пациентам выполняли ультразвуковое триплексное сканирование артерий каротидного бассейна. Ультразвуковую морфологию АСБ оценивали по нескольким ультрасонографическим признакам: однородность эхоструктуры и эхогенность. Нами использована классификация G. Geraulakos et al. (1993). Эндотелиальную скорость сдвига в общей сонной артерии определяли в соответствии с законом Хагена – Пуазейля.</p></sec><sec><title>Результаты</title><p>Результаты. У большинства пациентов (30 человек) были выявлены АСБ I типа, с несколько меньшей частотой встречались АСБ II (15 человек) и III (20 пациентов) типов. Однородные эхопозитивные бляшки, соответствующие «стабильному» фенотипу, выявлены у 5 пациентов. У пациентов с АСБ I и II типов значения эндотелиальной скорости сдвига составляли 373 (305; 481) и 311 (282; 419) c–1 соответственно. В группах пациентов с АСБ III и IV типов эндотелиальная скорость сдвига была достоверно выше – 500 (429; 556) и 470 (440; 512) c–1 соответственно. Среди пациентов с АСБ, в которых преобладал гипоэхогенный компонент, было достоверно больше пациентов с сахарным диабетом 2-го типа (p=0,006).</p></sec><sec><title>Заключение</title><p>Заключение. У пациентов с каротидным атеросклерозом, имеющих по данным ультразвукового исследования сонных артерий АСБ с преобладанием гипоэхогенного компонента, выявлены достоверно меньшие значение каротидной эндотелиальной скорости сдвига в сравнении с пациентами, имеющими преимущественно гиперэхогенные АСБ. Среди пациентов с нестабильными АСБ и низкими показателями эндотелиальной скорости сдвига в сонных артериях было достоверно больше пациентов с сахарным диабетом 2-го типа. </p></sec></abstract><trans-abstract xml:lang="en"><p>Aim – to investigate the relationship between the values of the carotid endothelial shear rate and ultrasound morphology of atherosclerotic plaques in carotid arteries.</p><sec><title>Material and methods</title><p>Material and methods. The study involved 70 patients with carotid atherosclerosis, including 43 males and 27 females. The average age of patients was (61.1±8.54) years. All patients underwent ultrasound triplex scanning of carotid arteries. Ultrasonic plaque morphology was evaluated using several ultrasonographic characteristics: uniformity of echotexture and echogenicity. We used the classification of G. Geraulakos et al. (1993). Endothelial shear rate in the common carotid artery was determined in accordance with the law of Hagen – Poiseuille.</p></sec><sec><title>Results</title><p>Results. Most patients (30) had a plaque type I, less frequently occurred plaque II (15) and III (20 patients) types. Homogeneous echo positive plaques corresponding to «stable» phenotype were found in 5 patients. In patients with plaque type I and II, endothelial shear rate values were 373 (305; 481) s–1 and 311 (282; 419) s–1 respectively. In the groups of patients with plaque– type III and IV endothelial shear rate was significantly higher – 500 (429; 556) s–1 and 470 (440; 512) s–1 respectively. Among patients with plaque with a predominance of echolucent component, more patients with type 2 diabetes mellitus were found (p=0.006).</p></sec><sec><title>Conclusion</title><p>Conclusion. Carotid endothelial shear rate was significantly lower in patients with carotid atherosclerosis and echolucent plaques compared to those having predominantly hyperechoic plaque.
