В структуре интерстициальных заболеваний легких лекарственно­индуцированные поражения легких встречаются приблизительно в 3 % случаев. Одним из наиболее пневмотоксичных препаратов, применяемых в кардиологии, является Амиодарон. Его токсическое действие многокомпонентно и влияет на все структуры бронхолегочной системы.</p><p>Цель – рентгенорадиологическая диагностика и динамическое наблюдение сосудистых нарушений в легких у пациентов с амиодарон­индуцированной легочной токсичностью (АИЛТ).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проанализированы результаты комплексного клинико­рентгенорадиологического исследования 110 пациентов, имевших прием Амиодарона в анамнезе. По данным комплексного исследования – диагноз АИЛТ был подтвержден у 90 пациентов. Средний возраст больных составил 71 год (ж/м – 21/69). Период наблюдения – от 1 месяца до 10 лет. МСКТ выполнена всем пациентам, в 81 % случаев – в динамике, от 2 (34 %) до 5 и более раз (20 %). ОФЭКТ – у 52,2 % пациентов, из них в 38,8 % случаев в динамике (от 2 до 4 раз).</p></sec><sec><title>Результаты</title><p>Результаты. Были выделены три клинико­рентгенологические формы заболевания: острые, подострые и хронические. Острая форма наблюдалась в 3 % случаев, подострая – в 68 %, хроническая форма наблюдалась как при первичном обращении (23 %), так и при переходе из подострой формы АИЛТ (38 %). По данным ОФЭКТ, 70 % пациентов имели выраженные диффузные нарушения микроциркуляции, 30 % перфузионные нарушения умеренной выраженности, с сохранением или нарастанием степени выраженности выявленных изменений при динамическом наблюдении – в 71,4 % случаях и присоединением тромбоэмболии мелких ветвей легочной артерии – в 20 % случаев.</p></sec><sec><title>Выводы</title><p>Выводы. При приеме Амиодарона происходит поражение всех структур бронхолегочной системы. При динамическом рентгенорадиологическом исследовании выявленные изменения носят стойкий и необратимый характер, имеется склонность к фиброзированию и рецидивированию, перфузионные нарушения в легких на фоне лечения полностью не восстанавливаются. При подозрении на ТЭЛА наиболее информативным методом диагностики является ОФЭКТ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. In the structure of interstitial lung diseases, drug­induced lung lesions occur in approximately 3 % of cases. One of the most pneumotoxic drugs used in cardiology is amiodarone. Its toxic effect is multicomponent and affects all structures of the bronchopulmonary system.</p><p>Objective of the study was to estimate radiologic features and to follow up vascular disorders in the lungs in patients with amiodarone­induced pulmonary toxicity (AILT)</p></sec><sec><title>Material and methods</title><p>Material and methods. We included 214 CT exams of 110 patients with history of amiodaron use. AILT was confrmed in 90 cases. In 81 % of patients we repeated CT exams 2–5 times, with observation period from 1 month up to 10 years. The mean age of patients was 71 years (21 females, 69 – males). In 52 % of patients lung scintigraphy was performed, in 34 % with follow up (from 2 to 4 times). We included functional lung test and cardiac ultrasound in diagnostic plan.</p></sec><sec><title>Results</title><p>Results. Three clinical and radiological forms of the disease were identifed: acute, subacute, and chronic. The acute form was observed in 3 % of cases, the subacute (in 68 %), the chronic form was determined during the frst examination (23 %) and during the transformation from the subacute form (38 %). According to SPECT­CT data 70 % of patients had pronounced diffuse microcirculation disorders, 30 % of patients developed perfusion disorders of moderate severity.</p></sec><sec><title>Conclusion</title><p>Conclusion. There are persistent and irreversible changes in the lungs with AIPT in dynamic radiological examination, there is a tendency to fbrosis and recurrence and perfusion disorders in the lungs during treatment are not fully recovered.
