Ингибиторы фосфодиэстеразы-5 (ФДЭ-5) обладают селективным вазоплегическим свойством в отношении легочной ткани. Однако прямой эффект ингибиторов ФДЭ-5 на альвеолярную микроциркуляцию до сих пор не изучен. Целью данной работы являлось изучение in vivo эффекта ингибитора ФДЭ-5 на легочную гемодинамику и микроциркуляцию на модели крупного животного.
7 свиньям (весом 30 кг) была выполнена срединная стернотомия. Легочная микроциркуляция оценивалась при помощи ортогонально поляризованного интровитального видеомикроскопа CytoscanTM (OPS imaging). Протокол острого эксперимента длительностью 30 минут предусматривал оценку параметров гемодинамики и микроциркуляции до введения препарата (BL) и после введения 0,15 мг/кг водного раствора ингибитора ФДЭ-5 непосредственно в легочный ствол.
В ответ на введение ингибитора ФДЭ-5 был отмечен эпизод снижения системного давления и давления в легочной артерии (18±0,9 с BL 21,5±0,8 мм рт. ст., р&lt;0,05) с тенденцией к компенсаторному восстановлению параметров. Легочно-сосудистое сопротивление достоверно понизилось с 10-й минуты эксперимента (136,0±13,8 с BL 172,8±16,9 в дин*с*см-5, р&lt;0,05). Достоверно увеличилось количество капилляров с диаметром меньше 9 мкм (67,0 % с BL 88,1 % к общему количеству, р&lt;0,001). Суммарная длина капилляров на одну альвеолу увеличилась в 2 раза (486,3 мкм с BL 240,8 мкм, р&lt;0,001). Отмечено увеличение диаметра альвеол (91,2 мкм с BL 85,1 мкм, р&lt;0,05). Индекс капиллярного кровотока увеличился (249,2 мкм/мкм2 с BL до 515,0 мкм/мкм2 р&lt;0,001). Скорость движения эритроцитов снизилась (245,5±63,5 с BL 205,7±74,3 мкм/с, р&lt;0,001).
Был впервые продемонстрирован эффект ингибитора ФДЭ-5 на микроциркуляцию легочно ткани in vivo. Изменения микроциркуляции капиллярной сети альвеол позволяют выдвинуть гипотезу о том, что ингибитор ФДЭ-5 вызывает тотальную релаксацию прекапиллярных сфинктеров, тем самым качественно меняя кровообращение на уровне капиллярной сети альвеол, и объясняет причину снижения легочно-сосудистого сопротивления.
