Широкая распространенность этого заболевания, в особенности сахарного диабета 2 типа (СД2), делает проблему профилактики острых сердечно-сосудистых катастроф у пациентов с СД2 весьма актуальной. Целью исследования явилось изучение прямого влияния сахароснижающего препарата метформина на ишемическое-реперфузионное повреждение миокарда как в эксперименте, так и в клинической практике. В экспериментальной части исследования было проведено сравнение двух путей введения метформина: системного хронического и локального однократного введения после моделирования острого ишемического повреждения. В результате выявлена возможность кардиопротективного действия препарата при его локальном интракоронарном введении животным без сахарного диабета. Клиническое нерандомизированное наблюдательное исследование не показало значимого защитного эффекта метформина в отношении ишемического-реперфузионного повреждения миокарда, возникающего у пациентов, оперированных в условиях искусственного кровообращения.</p></abstract><trans-abstract xml:lang="en"><p>The problem of cardiovascular complications of the diabetes mellitus (DM) draws close attention within the last decades. Patients with type 2 diabetes have 2-4 times greater risk for cardiovascular morbidity and mortality. Due to high prevalence of diabetes mellitus type 2, prevention of cardiovascular events in these patients is relevant. The goal of this study was to examine direct cardioprotective effect of antidiabetic drug metformin on ischemia-reperfusion injury in animal models and in patients. Two routs of metformin administration in acute ischemic injury were compared: continuous systemic before the ischemia and single local in the period of reperfusion. The cardioprotective phenotype appeared in local intracoronary administration to healthy animals.
