Abstract
Objective: To assess the significance of the research of peripheral skin microcirculation for the investigation of hypotensive drugs effect in patients with arterial hypertension (AH) at the microcirculatory (MC) level. Skin microcirculation (MC) was assessed by laser Doppler flowmetry method in patients with AH degree II-III before and after single dose of corinfar 20 mg (n=42), capoten 50 mg (n=38) and concor 5 mg (n=39). Decrease level of BP coincided with unidirectional shifts in skin microcirculation after different hypotensive drugs. Shifts in skin microcirculation parameters were characterized by increase in the level of tissue hemoperfusion caused by increased pulse blood filling and similar changes of functional components in micro-flow control. The data obtained show along with drug action also microcirculatory modifications coursed by significant reduction of arterial hypertension in altered hemodynamic conditions. The research of skin microcirculation demonstrates peripheral hemodynamics as a whole, therefore, its hardly probable to reveal the mechanism of hypotensive effect of drugs at the microcirculatory level due to its organ specificity.

Abstract
Correlation between metabolic and microhemodynamic processes in skin was assessed through acute pharmacological test with metabolically active Actovegin in 28 healthy volunteers. Laser Doppler flowmetry in combination with wavelet analysis of blood flow oscillations was used to identify functional state of arteriolar-venular areas of microvascular bed in the right forearm skin; capillary blood flow parameters were assessed through computer capillaroscopy in the nail bed of the right hand 4th finger. The metabolic effect (improved oxygen uptake and glucose disposal by tissues) was accompanied by significant increase vasomotion in endothelial rhythm amplitude by 98 % (p<0.00006), neurogenic rhythm amplitude by 50 % (p<0.003) and myogenic rhythm amplitude by 54 % (p<0.03), with capillary blood flow rate increasing by 90 pm/s (p<0.04), pericapillary zone reducing by 15 pm (p<0.0001) and diastolic blood pressure dropping by 4 mmHg (p<0.02). These results show close correlation between metabolic and microhemodynamic processes, which suggests that the amplitude activity within the range of endothelial rhythm (0.0095-0.021 Hz) during laser Doppler flowmetry does not reflect solely vasomotor but also metabolic function of microvascular endothelium.

Abstract
In case of disc herniated appearance it's often compression of radicular nerve happens, which accompanies with different clinical manifestation and microvascular disorders. In clinical practice of specialists dealing with sciatica it's common to use medications that take effect on microvascular regulation. Investigation of of pathophysiologic aspects of this effect is actual aim of number specialists facing with that question. Pupose - intraoperative evaluation of directions to pharmacologic correction of intraradicular microcirculation before and after surgical discectomy in sciatica patients. In this study were included 87 adult people with clinical manifestation of radiculopathy. During the standart Caspar's surgical discectomy the microvasular oscillations were measured from the radicular nerve using laser Doppler flowmetry method (LDF). All the patients were divided in 2 groups: I - control group - (56 adults - 22 females and 36 males) in which measures were taken before and after discectomy, without medications injection. II group (31 adults - 19 females and 12 males) - in which oscillations were measured before decompression, after intravenous injection of one medication: IIE (14 patients) - euphyllin, IID (11 patients) - dexamethasone and IIR (6 patients) - 30000 ME dextrane (rheopolyglucine), and after discectomy. Quantitative evaluation was assessed with microcirculation rate (MR, in perfusion units, p.u.) and qualitative evaluation with means of standardized amplitude of neurogenic sympathetic (An, 0.02-0.046Hz), myogenic (Am, 0.07-0.15Hz), respiratory venous (Ar, 0.15-0.4Hz) and cardial (Ac, 0.8-1.6Hz) rhythms. All the investigated medications showed their intraoperative influence on microcirculation. The most effective medication increasing MR and perfusion pressure was the group of middle molecular dextrane - rheopolyglucin.

Abstract
Investigated 109 patients (83 women and 26 men) aged 15 to 44 years in the closest (1 year) and remote (3 years) periods after active orthodontic treatment. Examined microcirculation indicators of gingival tissues by methods laser doppler flowmetry (LDF) and optical tissue oximetry (OTO). Revealed the reduction of averages LDF and OTO indicators in the age aspect, and during growth of degree manifestation of relapse. The lowest average rate of microcirculation (M) 8.6±1.17 (p<0.001); index of blood saturation (oxygenation) (SO2) 31.7±3.16% (p<0.001); volume fraction of tissues hemoglobin (Vr) 7.2±1.63 noted in the oldest age group (35-44) in patients with severe relapse in distant time (3 years) of retention period.

Abstract
We investigated the peripheral microcirculation blood flow (MBF) by laser doppler flowmetry as well as indicators of the right and left ventricles of the heart by echocardiography during an orthostatic test in 140 patients with chronic lower limb ischemia (CLLI) stage IIB-IV classification Pokrovsky-Fontaine. Objective: to study the state of the peripheral MBF and ventricles of the heart in functional tests in patients with CLLI before and after revascularization of lower limb arteries. It was found that patients with CLLI stage III-IV most pronounced disorders of functional reserves MBF as compared with patients with CLLI stage IIB before operation and in the immediate period after revascularization. We found that in patients with CLLI stage IIB and CLLI stage III-IV during an orthostatic test revealed no significant changes in the parameters of the right and left ventricles of the hear before operation and after revascularization of lower limb arteries.

Abstract
In article present data about research of biomarkers of dysfunction of an endothelium at patients with the thermal trauma which has become complicated multiorgan failure syndrome or which has not become complicated by development are presented. It is taped, that at the recovered patients at whom burns disease have become complicated development of multiorgan failure syndrome, excessively high concentration of factors of dysfunction of an endothelium, and at the lost patients - lower, than at patients without multiorgan dysfunction took place. It allows to draw a conclusion on excessiveness of response of an endothelium on a trauma in the first case and insufficient in the second. The given markers can be used for early diagnostics of a multiorgan failure syndrome and its correction.

Abstract
Article submittes data about influence of renal dysfunction caused by pre-existed renal diseases as well as macro- and microcirculatory disorders due to acute coronary syndrome with signs of stunning and hibernation for character, deepness of lesion and mortality in AMI. 485 hospital charts of patients suffered of AMI treated in Cardiology Clinic of SPb Institute for Emergency Medical Care of I. I. Dzhanelidze. Patients were divided on two groups - first one aged less then 60 years old included 161 patients, second one - aged more then 60 years old - 324 patients. Authors concluded that patient with AMI and elevated creatinine level significantly prevale in senior aged group, preferrably in patients with NSTEMI. In patients with NSTEMI and elevated serum creatinine Non-Q AMI occurred with higher rate and Q-MI met in the same cohort with STEMI with greater rate too (p<0.05) At the same time elevated level of serum creatinine had no effect to occurrence of recurrent AMI independently of patient's age. Mortality was greater inpatients of the second group with NSTEMI and elevated creatinine level (p<0.05).

Abstract
The effect of intravenous VRV-0411 and VRV-0511 administration on adrenoreactivity of rat mesentery microvessels was investigated in male Wistar rats. It was shown that both VRV-0411 and VRV-0511 decrease the vascular reactivity to Noradrenaline that can be explained by the possible changes of initial tone of the microvessels caused by VRV-0411 and VRV-0511.

Abstract
The purpose of this review is to choose the best experimental animal model of chronic obstructive pulmonary disease (COPD) in order to investigate the effects of smoking and chronic inflammation on the β-adrenergic receptors, M-cholinergic receptors, the mRNA expression of β2-adrenergic receptors and their sensitivity to the drug effects. The practical significance of the proposed work is in preclinical testing of alternative regimens, requiring prolonged use of β2-agonists and cardio selective β1-blockers, in an experimental animal model of COPD before clinical application of the management of patients with concomitant cardiopulmonary pathology, which is shown receiving a selective β2-agonists and cardioselective β1-blockers.

Abstract
In acute experiments on 23 male Wistar rats a direct detection of blood pressure in the carotid and femoral arteries was provided before and after intravenous infusion of dextran solution and acute blood loss. Harmonic analysis was applied to the resulting pulse wave curve for the estimation of generalized (GTF) and individual transfer functions (iTF) in concern with changes if the circulating blood volume. Analysis of the results showed that in a case of fluid volume load or alternatively a withdrawal of blood from the vascular system, the use of GTF for management central blood pressure according to the study of peripheral vascular pressure is impossible because of a large estimation error revealed by the comparison of the pressure curves and absolute values of the blood pressure, measured directly or reconstructed with the use of GTF. Application of iTF for the reconstruction of central blood pressure curve according to the study of peripheral vascular pressure is acceptable due to lack of a statistically significant difference between estimated pressure curves and directly measured absolute values of blood pressure.
